Harbor Care Health and Wellness receives distinction
BEDFORD – Nashua’s Harbor Homes’ Harbor Care Health and Wellness Center, with support from New Hampshire Healthy Families, has been recognized as a Patient Centered Medical Home by the National Committee for Quality Assurance. This recognition distinguishes Harbor Homes for its commitment to high quality primary and preventive patient care.
Achieving Patient Centered Medical Home recognition means that Harbor Homes’ health center is meeting high standards of patient care including same day appointments, focused attention from health professionals, proactive support for patients with chronic illnesses, and effective systems to link patients to specialty care. The overall goal is to improve access to primary care services and enhance care coordination.
Working closely with Harbor Homes, New Hampshire Healthy Families provided support to implement the changes that were necessary to transform the practice into a Patient Centered Medical Home.

New Hampshire Healthy Families has a dedicated manager on staff whose focus is to help primary care practices achieve National Committee for Quality Assurance certification as Patient Centered Medical Home. New Hampshire Healthy Families recognizes that busy practices may not have the time or expertise to implement and maintain the processes to meet the National Committee for Quality Assurance standards. Services include formulating a road map to Patient Centered Medical Home recognition, helping with document submissions, and providing tools and training.
This resource is available to all New Hampshire Healthy Families network providers.
