Merrimack kindergarten, first-grade registration information

Superintendent of Schools Marjorie C. Chiafery announced registration information for all Merrimack children entering public school kindergarten and grade one in the 2016-17 school year.

To be eligible for kindergarten registration, a child must be 5 by Sept. 30, 2016. To be eligible for first-grade registration, a child must be 6 by Sept. 30, 2016.

The registration process will begin with parents downloading and completing the New Student Information Entry Form available on the Merrimack School District website at Parents should return the form along with the following information to their neighborhood school from Monday, Jan. 11, through Friday, Jan. 15, between 9 a.m. and 2 p.m.

n Child’s original birth certificate with the raised seal (a copy will be made for records and the original returned immediately).

n Immunization record.

n Copy of most recent medical examination (within one year of enrollment year).

n Proof of residency (utility bill or lease agreement and/or mortgage agreement).

n If applicable, legal papers.

Entry forms will also be available at the school for those who do not have access to a computer.

All students registering for public school for the first time are required by state law to have a physical examination by a licensed physician. This exam must take place within one year prior to the first day of school.

The child is also required to be immunized for the following before entering school: measles, mumps, rubella, diphtheria, pertussis and tetanus, oral and/or injected polio, varicella and hepatitis B vaccine. The month, day and year are required for all immunizations.

No child will be allowed to enter school in the fall unless he or she has met all of the above requirements. Parents are urged to complete the physical examination and immunizations as soon as possible, as area doctors often have difficulty scheduling appointments late in the summer. Please have your child’s medical record completed and returned to school by Monday, Aug. 1.

The Nashua Public Health Department has immunization clinics for young children every week. Immunizations are available for a small fee. The center is located at 18 Mulberry St. For the clinic schedule and directions, call 589-4560.
